Providing Self Directed
Support Services Since 2012
Who We Support
Oregon Provider primarily supports individuals seeking in-home, community-based, and employment services. We are contracted with the Oregon Department of Disabilities Services, Vocational Rehabilitation, and the Oregon Commission for the Blind.
We proudly provide services throughout Clackamas, Multnomah, and Washington Counties. Our Activities Director offers daily opportunities that help individuals build friendships, strengthen community connections, and participate in safe, meaningful activities.
Our Direct Support Professionals and Employment Professionals are trained as skills trainers, using a teaching-focused approach that promotes independence, confidence, and personal growth.
We believe in consistent, routine-based services, scheduling clients on the same day and time each week whenever possible. Our agency values communication, collaboration, diversity, advocacy, and self-directed supports.
Our employment support services help individuals explore, prepare for, find, and maintain meaningful employment.
We support each person throughout the full employment process, including Discovery, Career Exploration, Job Development, and short term or long term Job Coaching.
Our goal is to help each individual build confidence, develop skills, and become as independent as possible in the workplace. Our Employment Professionals bring a wide range of experience and are dedicated to providing the tools, guidance, and support each person needs to succeed.
